HealthStone PLUS is marketing website that was developed to promote the services of like-minded health care providers. In today’s ever changing healthcare landscape, there is a push towards providers working closer together with other providers in the healthcare continuum to form coordinated, high-value healthcare models that have aligned incentives. A successful alignment strategy is predicated on dedicated healthcare providers working together to constantly share information and collaborate on meeting unique patient needs. All too often, healthcare has become fragmented and the patients are the ones left to sort through the confusion. In a coordinated model, the patient is the focal point and the providers work together to ensure that the needs of the patient is being met. The patient is a critical part of the decision making process and their caregivers play an important role in the development and administration of comprehensive care plans.

HealthStone PLUS is a tool for patients, providers and consumers of healthcare services to be able to identify and locate providers in their community. All of the providers who market on this website have agreed to work together to assist in the coordination of care for their patients and to promote the principles of the ‘Triple-Aim’ that were developed in the Affordable Care Act. Through patient engagement and education, we can improve the health of the community one patient at a time.
